CV for the future recruitment processes

I hereby consent to the processing of my personal data provided in the recruitment application by Medicalgorithmics S.A., with its registered office in Warsaw for the purposes of this recruitment process.

The data controller is Medicalgorithmics S.A. based in Warsaw (02-001) at Al. Jerozolimskich 81, nr tel: +48 22 825 12 49. The Administrator has appointed a Data Protection Officer with whom you can contact at the email address: compliance@medicalgorithmics.com. The data will be processed for the purposes of conducting the recruitment process and, in case of giving the appropriate consent for the purpose of conducting recruitment processes in the future, whereby the legal basis for the processing of personal data is primarily art 6 para. 1 point c in connection with art. 221 of the Labour Code, as well as art. 6 ust. 1 point a of GDPR, in the case of special category personal data art. 9 para. 2 point a of GDPR, which is consent to the processing of personal data contained in the CV or in other documents that the Candidate has provided to the employer, and which are not explicitly required by labor law regulations With regard to personal data processed on the basis of consent to data processing, the Candidate has the right to withdraw their consent at any time without affecting the legality of the processing carried out on the basis of consent before its withdrawal. The consent can be withdrawn at the following address: compliance@medicalgorithmics.com. The administrator will process your personal data, also in subsequent recruitments, if you give your consent, which can be revoked at any time. Providing data is a condition for participating in the recruitment process, and failure to provide it will prevent participation in the process. Providing other data is voluntary The data will be stored until the end of the recruitment process. In case you give your consent for the use of personal data for future recruitment purposes, your data will be stored for a period of 24 months. You have the right to: access your data and receive a copy of it; rectification (correction) of your personal data; restriction of processing personal data; erasure of personal data; filing a complaint to the President of the Personal Data Protection Office (at the address of the Office for Personal Data Protection, Stawki 2, 00-193 Warsaw) Only authorized employees have access to your personal data, as well as entities participating in the recruitment process with whom Medicalgorithmics S.A. has entered into appropriate agreements. Your personal data may also be transferred to entities that are authorized to receive them by law. Additionally, they may be disclosed to entities with which Medicalgorithmics S.A. has entered into an agreement for the provision of service services for the systems/applications used in their processing (Traffit Sp.z o. o., Legionów street 112/21, 81-472 Gdynia).
